Not tightening the barrel nut all the way definately makes cartridges seperate, to hot of ammo can do the same thing, cartridges that have been loaded to many times as well.
Failure to put the nut on at all like some have done will launch the barrel about 25 yards. This generally leads to a scope growing out of your forehead. I don't believe this would feel very good.
MAKE SURE YOU INSTALL AND TIGHTEN YOUR BARREL NUT POSITIVELY.
